2007-2008 SAFE Award Scholarships
What is the Kentucky Safety and Health Network Foundation?
The Kentucky
Safety and Health Network Foundation (KSHNF), Inc. is a sister organization to
the Kentucky Safety and Health Network (KSHN). The KSHNF is a private,
non-profit corporation operating within the Commonwealth of Kentucky. The
Foundation’s purpose is to increase the level of occupational safety and health
knowledge in the Commonwealth by awarding scholarships and educational research
grants.
What are the KSHNF
“SAFE” Award Scholarships?
Annually, the
Foundation provides scholarships to eligible undergraduate and/or graduate
students attending a four (4) year university. The scholarships are known as
the KSHNF Scholastic Achievement For Education Awards, or “SAFE” Awards. SAFE
awards are presented in the disciplines of occupational safety and health,
industrial hygiene, and public health.
Who Is Eligible to
Receive KSHNF SAFE Awards?
AT TIME
OF APPLICATION:

A student must be enrolled in one (1) of the disciplines stated
above;

Undergraduate students must have completed a minimum of sixty (60)
semester hours;

Undergraduate students must have at least an overall grade point
average of 3.0 out of a 4.0 system;

Graduate students must have completed or be carrying at least nine
(9) semester hours;

Graduate students must have at least an overall grade point average
of 3.5 out of a 4.0 system for all graduate work and at least an
overall grade point average of 3.0 out of a 4.0 system for all undergraduate
work;
- All applicants must be a Kentucky resident attending a
Kentucky university or out of state university and be able to provide a
permanent Kentucky address; or, a non-Kentucky resident attending a Kentucky
university.
Application
Requirements
An applicant must submit:
- A completed application form.
- A resume.
- Two (2) letters of academic recommendation from the major
field of study.
- A letter
from the department chair or applicant’s faculty advisor attesting to the
applicant’s admission in a program of one (1) of the aforementioned
disciplines.
- An original essay describing the applicant’s career goals and
future intentions in the profession of occupational safety and health,
industrial hygiene, or public health.
The essay must be double spaced and have one (1) inch margins. The essay
must be a minimum of two (2) pages and no more than four (4) pages.

A certified complete transcript for all
institutions attended and coursework completed. For graduate applicants,
complete transcripts must also be provided for all undergraduate work. For
all applicants, although the Foundation prefers that the transcript be sent
directly by the institution to the address below, the applicant may submit a
certified transcript(s). Photocopies of the transcript(s) will NOT
be accepted.
Submission Criteria
- The SAFE application may be submitted online anytime via the
link below.
- The resume and essay may be submitted in MS Word as email
attachments to scholarship@kshn.net. If submitted via email, they must be
submitted together in one (1)
email.
- All application
material not submitted electronically must be submitted to the address below
in one (1) mailing, except transcripts sent directly by the
institution. If preferred by the authors, the two (2) letters of
recommendation and/or letter attesting to applicant’s admission in a program
of one (1) of the aforementioned disciplines may be sealed in a separate
envelope within the applicant’s single portfolio submission.
Deadline
All applicant
materials must be postmarked or received electronically by midnight March 1,
2008. There are no exceptions. Portfolios that do not meet the aforementioned
criteria or postmarked or received electronically after March 1, 2008 will be
invalid and not considered for an award. Neither the Foundation nor the KSHN
will notify the applicant of incomplete portfolios. It is the applicant’s
responsibility to ensure the portfolio is complete.
Awards
SAFE Award
recipients will be announced and contacted by April 30, 2008 and recognized at
the May 2008 Kentucky Governor’s Safety and Health Conference and Exposition in
Louisville. The actual award in the form of a check will be provided to the
department chair
at
the recipients next semester of instruction. Students wishing to use the
scholarship for summer school tuition must notify the KSHN Foundation as soon as
possible upon enrolling for the summer term. The Foundation reserves the right
to withdraw all awards if the recipient does not return to school.
