Please provide all information below exactly as you wish it to appear in the conference program. Workshops are one (1) hour and fifteen (15) minutes in length which allows for one (1) hour of instruction time. Workshop proposals are due June 15.
If you have multiple speakers, please complete the initial proposal in full and submit subsequent proposal forms for each speaker.
